Follow the instructions on the Adobe Web site for downloading and installing. Then return to this page for instructions on downloading, filling out, and printing your new-patient forms.

After you have Adobe Acrobat Reader installed, use your right mouse button to click here: Download New-Patient Forms In the menu that pops up, select "Save As", and then save the PDF file to your computer (remember here you saved it so that you can open it later). Start Adobe Acrobat Readfrom your start menu, and then open the PDF file which will open the New-Patient form. In the document that appears fill out the required information. Once you have finished filling out the form, select print from the file menu and bring the printed form to your first visit! Please note that you cannot save a the filled out form to your computer.
